FAVE Enrollment Information and Interest Form

Dear Prospective FAVE Families,

Thank you for your interest in Fulton Academy of Virtual Excellence (FAVE).  We are the full-time virtual school within the Fulton County School District.  The Spring 2025 enrollment window is open between December 9, 2024 to January 15, 2025.  Please read through our instructional page and complete all the required steps for submitting your student’s application.  Families should not withdraw their students and students should continue attending their current school until they have received communication from our registration team to do so.

It is important to gather all of your documents prior to beginning your application. Reference the list below for support. You will need to upload these documents to complete and submit the application.

·       Most recent Progress Report - saved as Student Last Name_Progress Report

·       Most recent Transcript (Middle & High School) / Report card (ES) - saved as Student Last Name_Transcript

·       Most recent Attendance Report - saved as Student Last Name_Attendance

·       IEP (If Applicable) – email to FAVE IST Ashanti Johnson johnsona18@fultonschools.org

·       NEW Fulton Families Only - Completed Records Request Form (attached to this email)

 

Reports can be found in the Campus Parent Portal (for current FCS families), or you may contact your current school for academic records. Please save files using the following naming conventions: Example for multiple pages - Transcript_Student Last Name_1, Transcript_Student Last Name_2, etc.

·       Enrolling Parent/Guardian Driver's License or State Issued ID

·       Two Proofs of Residency (one from the approved utility list and one from the approved residency list. Click here for the approved list of residency documents.)

·       Child's Birth Certificate

·       Child's Social Security Card

·       Most current Georgia Ear, Eye, Dental, & Nutrition Screening Form

·       Georgia Immunization Certificate

Failure to provide academic and attendance records will cause a delay in the processing of your application.
