FAVE Application Information and Interest Form
FAVE serves students in grades 6 through 12. Spaces for each grade level are limited for interested students. Before you begin, please understand that you will need to complete TWO SEPARATE FORMS. Our FAVE Qualtrics application form AND the Fulton County Online Registration. You must submit ALL documents for your application to be considered.
Please remember to remain enrolled at your current school while your application is being reviewed. Students must be accepted into FAVE in order to be enrolled.

Prior to applying at FAVE, please collect your documents and properly save your files to your computer/mobile device. Applicants will need several documents to complete the FAVE Application. We've provided a list of documents below so that you can gather your documents before starting the application. (OLR = Fulton County Online Registration Application)
FAVE Application Documents: Reports can be found in the Campus Parent Portal (current FCS families), or you may contact your current school for academic records.

- Most recent Transcript - saved as Student Last Name_Transcript
- Most recent Attendance Report - saved as Student Last Name_Attendance

Fulton County Online Registration Application
- Enrolling Parent/Guardian Driver's License or State Issued ID
- Two Proofs of Residency
- One from the approved utility list and one from the approved residency list.

- A copy of the enrolling parent/guardian's driver's license or state-issued ID
- Child's Birth Certificate
- Child's Social Security Card
- Georgia Ear, Eye, Dental, & Nutrition Screening Form
- Georgia Immunization Certificate
